Similar Appearance, Different Anatomy
Because the eyebrows and upper lids sit close together, changes in either area can make the eyes appear tired or crowded. When the brow descends, skin from the forehead can settle closer to the eyelid crease. Loose upper-lid skin or protruding fat may create a similar appearance even when the eyebrows remain in a natural position.
A complete facial plastic surgery evaluation can clarify where the hooding begins. This distinction matters because each surgery treats a different part of the upper face.
When a Brow Lift May Be More Appropriate
A brow lift may be considered when the eyebrows have moved downward and are contributing to fullness above the eyes. Other signs can include a low outer brow, pronounced forehead lines, or tissue that seems to press against the upper lids.
The procedure repositions the eyebrow and forehead tissues rather than removing skin directly from the eyelid. Treatment planning accounts for facial structure, brow placement, and the degree of tissue descent.
When Blepharoplasty Treats the Source
Blepharoplasty works directly on the eyelids. It may be a better match when excess skin folds over the natural upper-lid crease or localized fat creates puffiness. During upper eyelid surgery, the incision is placed within the crease so excess skin, muscle, or fat can be carefully removed or repositioned.
Age alone does not determine eyelid surgery candidacy. Inherited features, skin quality, general health, and the location of the unwanted tissue all influence the recommendation. For concerns such as fine lines or mild laxity, we may also discuss surgical and nonsurgical eyelid treatments.
When Combining the Procedures May Help
Some patients have both eyebrow descent and excess upper-lid skin. Correcting only one area may leave the other source of hooding unchanged. In those cases, combining a brow lift with blepharoplasty can treat the forehead and eyelids within the same surgical plan.
This type of coordinated planning may also be useful when combining facial procedures to improve several related signs of aging while maintaining balanced facial proportions.
